The MDS Coordinator is responsible for the accurate and timely completion of the Minimum Data Set (MDS) for all residents in the facility. The coordinator reviews resident assessment information to ensure that all required data is collected and entered into the MDS system. The coordinator works with the interdisciplinary team to develop care plans that reflect changes in resident condition. The coordinator also provides education to staff on MDS completion and documentation requirements.

MDS Coordinator Job Responsibilities

  • Develop and maintain the MDS schedule.
