The Continuous Care LVN is responsible for providing direct patient care to assigned patients in the home setting. The Continuous Care LVN will make frequent visits to patients’ homes in order to provide skilled nursing care and support. The Continuous Care LVN will collaborate with the patient’s physician and other members of the healthcare team to develop and implement a plan of care. The Continuous Care LVN must be able to work independently and be self-motivated.

Continuous Care LVN Job Responsibilities

  • Providing continuous care to patients with chronic or longterm health conditions
  • Monitoring and recording patient vital signs and other important health indicators
  • Administering medications and treatments as prescribed by the patient's physician
  • assisting patients with activities of daily living, such as bathing, dressing, eating, and toileting
  • educating patients and their families about their health condition, treatment plan, and selfcare measures
  • collaborating with other members of the healthcare team to provide coordinated care for the patient

Continuous Care LVN Job Skills & Qualifications Needed

  • A high school diploma or equivalent is required for the role of Continuous Care LVN. Completion of an accredited vocational nursing program and a valid state license are also necessary.
